Worker's
Compensation
Eligibility:
All Wartburg College employees are covered
Purpose:
To cover medical benefits for employees injured on the job
Procedure:
- Employees
who are injured on the job should immediately report the injury
to their supervisor, regardless how minor the injury.
- The Work-Related
Injury Report Form (PDF) should be completed at (or sent
to) the HR Office, 203 Luther Hall.
- If medical treatment is necessary,
designated providers are Allen Occupational Health, Iowa Health
Systems (formerly Rohlf Clinic), 8 a.m. - 5 p.m. Monday through
Friday, or the Waverly Health Center.
- The
doctor or hospital should be notified that the claim is for a
work injury and will be paid through Workers' Compensation, not
the employee's health insurance.
Premiums: Entire premium is
paid by Wartburg College
