to Researchers
1) Verify that your activity
requires HARRC oversight.

3) Download and complete Human
Subjects Protocol Form (DOC). Include all information in one
electronic file (MS Word or PDF) if possible. Type and/or paste
any additional forms (e.g., consent forms, survey questions, debriefing
forms, etc.) at the end of the protocol form.
4) If you are a student researcher,
send the electronic version of your completed form to your research
advisor. Your research advisor should review your protocol before
sending it to HARRC.
5) The research advisor should email
the completed protocol form to HARRC@wartburg.edu.
HARRC will not review protocol forms that are submitted directly
from students.
6) Print out and sign the signature
sheet on the protocol form and send it via campus mail to the HARRC
chairperson.
7) During the academic year HARRC
will meet weekly. To ensure a timely review of your protocol, send
your protocol form via e-mail to HARRC at least two days
before HARRC meets. Contact the chairperson of HARRC to find out
when HARRC meets this term.
